BACILLARY ANGIOMATOSIS: ETIOPATHOGENETIC, DIAGNOSTIC AND THERAPEUTIC APPROACH

Bacillary angiomatosis is an unusual systemic vascular proliferation, most often described in patients with A.I.D.S. or other causes of immunodeficiency. Skin lesions might resemle pyogenic granuloma or Kaposi's sarcoma. The usual causative organism Bartonella Henselae, has only recently been identified. Bartonella Quintana has also been shown to cause some cases of skin Β.Α. Light microscopical examination of skin biopsy specimens, is the most commonly used method of confirming the diagnosis. The most characteristic feature is lobular vascular proliferation composed of plump "epithelioid" endothelial cells. Neutrophils are scattered throughout the lesion, especially around eosinophilic granular aggregates, which can be seen to be masses of bacteria when examined by Warthin - Starry staining or by transmission electron microscopy. For the treatment of this disease, erythromycin is the drug of choice while excellent responses have also been obtained with doxycycline. Α number of other antibiotics has been successfully used in limited numbers of patients.
